Quick answer: In the US, BetterHelp and Talkspace both accept many insurance plans; Talkspace also offers psychiatry, and self-pay prices for all three run about $60–120 a week. Online-Therapy.com bundles a structured CBT programme but takes no insurance. In the UK and Canada, Talkspace works only with US-licensed therapists, so BetterHelp or Online-Therapy.com are the practical options, after free routes such as NHS Talking Therapies.

We're paid a commission by all three companies when someone signs up through our links, so this comparison sticks to what each publishes about price, insurance, sessions and privacy, and to the public record. We don't rate them on clinical results: there's no independent head-to-head evidence that one produces better outcomes than another.
Who this comparison is for — and when an app isn't the right start
It's for adults who have decided they'd like talking therapy, can't get it quickly enough through a free route, and want to know which subscription service fits where they live and what they can pay. None of the three is a crisis service. If you're thinking about suicide or feel unsafe, call or text 988 in the US or 9-8-8 in Canada, call Samaritans on 116 123 in the UK, or call 911 or 999 in an emergency.
An app is also the wrong first step if you need a diagnosis, medication outside the US, help with an eating disorder or alcohol or drug withdrawal, or documentation for a court or employer. BetterHelp lists several of these as things it can't do; a GP, family doctor or primary-care clinician can.

In the US, insurance changes the maths
If you have private insurance, start by checking your plan, because two of the three now bill insurers directly. BetterHelp says it is in-network with more than 125 US plans, including Aetna, Cigna, UnitedHealthcare and some Blue Cross Blue Shield plans, but not Medicare, Medicaid or TRICARE. Talkspace works with many plans and puts the average co-pay at $10 a session; it also accepts traditional Medicare in certain states, but not Medicare Advantage. Online-Therapy.com doesn't take insurance; its FAQ says it provides receipts you can submit for out-of-network reimbursement.
Talkspace changed hands this year: hospital operator Universal Health Services completed its purchase in August 2026, according to Becker's Hospital Review, and plans to connect Talkspace members with its own services. That doesn't change your therapy, but it's worth knowing who owns the company.
Without insurance, the three cost broadly the same per week. Before paying, check whether a federally funded health center near you offers counselling on a sliding scale by income, or whether your employer has an assistance programme with free sessions.
In the UK and Canada, there are fewer real choices
Talkspace matches you with therapists licensed in US states. Its help centre says self-paying members can use it from abroad, but most providers work US hours, so it isn't a practical option for most people in the UK or Canada. That leaves BetterHelp, which charges about £35–75 a week in the UK and CA$85–110 a week in Canada, and Online-Therapy.com, run from Sweden by CRN Solutions AB, which prices in local currency at checkout. Online-Therapy.com asks UK therapists for registration with bodies such as BACP, UKCP or BABCP, and Canadian therapists for CCPA or CRPO credentials "or similar". BetterHelp's UK and Canada pages don't name the registers its therapists belong to, so ask your therapist directly.
The free route is worth trying first. In England, NHS Talking Therapies takes self-referrals without a GP; our step-by-step self-referral guide explains how. In Ontario, Ontario Structured Psychotherapy offers free CBT to adults by self-referral; other provinces run similar programmes.

Privacy and track record
All three handle sensitive information, so the public record matters. In July 2023 the Federal Trade Commission finalised an order requiring BetterHelp to pay $7.8 million after alleging it shared email addresses, IP addresses and health questionnaire answers with advertising platforms. BetterHelp disputed the allegations; the order bans it from sharing health data for advertising, and the FTC began sending refunds to about 800,000 customers in May 2024. BetterHelp's current privacy policy makes advertising cookies opt-in.
A proposed class action against Talkspace over a TikTok tracking pixel was filed in 2024 and withdrawn in 2025, according to Bloomberg Law; no court or regulator made findings. We found no regulator action against Online-Therapy.com, which says it deletes data after two years by default. Whichever you choose, read the privacy settings when you sign up and switch off anything optional.
How to choose
- US, insured: check which of BetterHelp or Talkspace is in your network; the co-pay usually beats any self-pay price.
- US, you may need medication: Talkspace is the only one of the three with prescribers, or see a primary-care clinician.
- You like structure and homework: Online-Therapy.com's CBT programme and worksheets are built in.
- UK or Canada: BetterHelp or Online-Therapy.com, after checking the free routes above.
- Budget is tight: try the free routes first; if you pay, Online-Therapy.com's messaging-only plan is the cheapest way in, but it includes no live sessions.
Most services let you switch therapist; BetterHelp's terms, for example, allow cancelling at any time, though unused paid sessions expire. If the first match doesn't feel right after two or three sessions, switching is normal.
What to do next
- If you're in crisis, use the numbers above rather than an app.
- Try the free route for your country — and join its waiting list even if you also pay for an app.
- Check insurance or an employer programme before paying full price in the US.
- Compare one month's real cost, including how many live sessions you'd actually get.
- Review after a month. If you're not feeling any better, or feel worse, talk to a GP, family doctor or primary-care clinician.
The mental-health support finder matches how much this is affecting you to self-help, online therapy, your doctor or crisis support, and shows free routes first.
Find my supportCommon questions
Does BetterHelp take insurance now?
Yes, in the US. BetterHelp says it is in-network with more than 125 plans, including Aetna, Cigna and UnitedHealthcare, but it doesn't accept Medicare, Medicaid or TRICARE directly. Without coverage it costs $70 to $100 a week, billed every four weeks. Check that your plan lists BetterHelp before you sign up.
Can I use Talkspace in the UK or Canada?
Not in the usual way. Talkspace matches you with therapists licensed in US states. Self-paying members can use it from abroad, but most providers work US hours, and insurance or employer plans don't apply outside the US. In the UK or Canada, BetterHelp and Online-Therapy.com are the practical choices.
Can an online therapist prescribe medication?
BetterHelp and Online-Therapy.com therapists can't prescribe. Talkspace offers psychiatry for adults in the US, with a first evaluation priced at $299, and it doesn't prescribe controlled substances. Elsewhere, medication comes from your GP, family doctor or a psychiatrist, which is one reason to see them first if you think you need it.
Is my information safe with these apps?
Each says it protects health data, but check the privacy settings when you join. In 2023 the FTC ordered BetterHelp to pay $7.8 million over sharing health questionnaire answers for advertising, which BetterHelp disputed; the order now bans that. We found no regulator action against Talkspace or Online-Therapy.com.
